If it is over 32gb it may not be recognized by the BB 6/7 OS. I was about to take my phone back as I thought it was defective.

i've inserted the card properly and its just not working. it could be partition setup that is causing the issue that or its just not inserted properly. I was trying to close the metal piece over the card. I have followed ALL of the steps listed above and it still says "MEDIA CARD FORMAT FAILED!"
